Sýni tekið í 2-4 ÍSKÆLD glös með fjólubláum tappa án gels (svört miðja)
Sett strax í kælikubb (ísvatn) og snúið STRAX niður í kældri skilvindu.

Sjúklingar ættu að vera fastandi (10 klukkustundir).
Stöðva ætti histamín 2 blokka eins og ranitidín í 72 klukkustundir og próteinpumpuhemla eins og omeprazol tveimur vikum fyrir þetta próf.

Hypercalcemia og nýrnabilun getur valdið hækkun á gildi gastríns
Magn: 3-6 mL
Geymsla sýnis: Frystir. Skilið innan við 15 min frá blóðtöku í kæliskilvindu og fryst síðan

Plasma verður að vera komið í frysti hálftíma eftir blóðtöku!

Gut hormone levels are indicated in the investigation of patients with symptoms suggestive of a neuroendocrine tumour or for screening of patients with multiple endocrine neoplasia type I

The profile includes the following gut hormones: 

    • Gastrin
    • Glucagon
    • Pancreatic polypeptide
    • Vasoactive intestinal polypeptide
    • Somatostatin
    • Chromogranin A and B
Neurotensin no longer forms part of the routine profile but is available in certain circumstances - please contact the Duty Biochemist if considered necessary.

CART (cocaine- and amphetamine related transcript) is also available if considered necessary - again please contact the duty biochemist.

 Sample requirements

    • Patients should fast overnight (10 hours).
    • Histamine 2 blockers such as ranitidine should be stopped for 72 hours and protein pump inhibitors such as omeprazole stopped two weeks before this test.
 For adults 2 x 4mL pre-chilled EDTA tubes on ICE

Storage/transport

 Do not store. Send immediately to the laboratory on ICE as the sample must be separated and plasma frozen within 15 minutes.

Relevant clinical details including current medication.

Turnaround times

The samples are sent frozen by courier on a weekly basis to the Hammersmith Hospital for analysis with results expected back within four weeks.

Fróðleikur:

A gut hormone screen is used for the diagnosis of a variety of endocrine tumours of the pancreas, ganglioneuromas, gastrinomas or G-cell hyperplasia. Analysis includes gastrin, VIP, somatostatin, pancreatic polypeptide, and glucagon.

The gastrin test is usually requested to help detect high levels of gastrin and stomach acid. It is used to help diagnose gastrin-producing tumours called gastrinomas, Zollinger-Ellison (ZE) syndrome, and hyperplasia of G-cells, specialised cells in the stomach that produce gastrin. It may be measured to screen for the presence of multiple endocrine neoplasia type I (MEN) It may be used if a person has abdominal pain, diarrhoea, and recurrent peptic ulcers. A gastrin test may also be requested to look for recurrence of disease following surgical removal of a gastrinoma.

Vasoactive intestinal peptide (VIP) measurement is required for diagnosis of pancreatic tumour or a ganglioneuroma which secretes VIP. Administration of VIP to animals causes hyperglycaemia, inhibition of gastric acid, secretion of pancreatic bicarbonate and of small intestinal juice, and a lowering of systemic blood pressure with skin flush. These features are seen in patients with a tumour of this type which is secreting VIP.

Glucagon is measured for preoperative diagnosis of a glucagon-producing tumour of the pancreas in patients with diabetes and a characteristic skin rash (necrolytic migratory erythema).

Pancreatic polypeptide (PP) production is most commonly associated with tumours producing vasoactive intestinal polypeptide and with carcinoid syndrome and, less commonly, with insulinomas and gastrinomas.

When secreted by endocrine tumours, somatostatin appears to produce symptoms similar to those seen on pharmacological administration, i.e. steatorrhoea, diabetes mellitus and gall stones.
